What can I gather to help get a approval by Healthcare USA Mo- medicade?

I want to gather as much info. as I can to help get approved by Healthcare USA- Missouri medicade.My PCP has a very small pratice and has not ever had someone who is wanting to have WLS. why dont you call them and ask what their requirements are. That way you and your Dr will have a head start at trying to meet those requirements. You could even see if your Drs office will make the call. Good Luck
