Have you checked with Med Mutual directly? Surely they can answer your question.

My husband is employed by the State of Ohio and he will be changing to Medical Mutual - Ohio Med PPO. I spoke to them on the phone just a couple of weeks ago and according to the lady I spoke to, they cover it and she gave their requirements. It could depend though on what your individual employer contracted with them. I would contact them directly and ask. Good Luck.

Any word on if they cover it this year?? I just got a packet from them and it states that all of their plans exclude "Surgery for weightloss". I remember last year, when I received a packet with info on applying it said, "Surgery for weightloss is excluded UNLESS deemed medically necessary" - but now it just says it's excluded. Anyone know???? They ask for my SSN everytime I call to ask even the simplest question... no way I'm divulging it!
