Question:
My doctor asked me to give him copies of my diet history to submit to Aetna

Check my profile for how to handle AETNA. They are great when they get the info they need. My hospital was out of network and AETNA paid 80% anyway (20,000!). Email me if you need any assistance. I used the sample letter from this site--fax it and mail one just to be on the safe side!
