Does anyone have a sample letter to their insurance Co.that they can share with me?
HELP!...my insurance company needs more medical documentation of previous weight-loss attempts...the only problem is that my old health records were lost when my health clinic closed.I am in the process of writing a personal letter and need sample letters. Fabiola, Hi! I have Cigna HMO and was denied on my first attempt. I then
resubmitted everything again but with an appeal letter. My letter was given
to the review board on Friday afternoon and by Monday at 9:00am they called
my with an approval. The power of a good appeal letter is great. I would be

My suggestion is to go to the professionals on this! (Not at all meaning
to diminish the importance that we weight loss surgery patients bring to

Did you try your state medical records usually be able to get copies of
your records there
