I would see if the surgeon's office can call and talk to whoever you talked to.   What number did they give you for provider inquiry.  Provider iNquiry will only speak to providers located in Michigan with a Michigan Provider number.

My provider is in Indiana since I live in Indiana. The provider inquiry # given to me was 800-344-8525. I talked to the woman who handles insurance in my surgeon's office today and she seemed like she thinks it's a total waste of her time to call the provider inquiry # that I was given.
