Cost of getting ohip forms done by doctor

I didn't pay anything. I filled out most of the form before I even went to see the doctor. He even wrote me a letter no charge. Linda

it shouldn't cost you a thing the spot to put the diagnostic code is on the first page down near the bottom along where your Dr. puts his address and the procedure you are wanting done Dannette

I did not pay a cent for mine to be signed. I think some people paid for a letter to by done by thier doctor. I didn't. I sent in the forms and a letter from myself. That was it! Talk to your doctor. Jen S.
