Unforunately, there may be no way around this one. They will not give you a basic exam because bariatric surgery doesn't just affect us physically. It affects us emotionally as well.
You can contact your former Bariatric Surgeon. They will have a copy of your approval from the last Psych Doc. But, since it was more than a year ago, your new surgeon will require you to have another eval. Or you could even contact your insurance company for history of their payments made?
The docs just want to make sure we can 'handle' the emotional aspects of bariatric surgery. I'm sure you are aware of this, being you've been there/done that.
Especially with a revision, there are other issues that come along with it.
Please don't underestimate the care you need.
Better to be overly cautious than not cautious enough.
Check out the area support groups too! You may be able to get recommendations from someone else who attends!

Definition: Hypolipidemia is a condition where cholesterol levels are too low in the body. Typically, no symptoms are associated with low cholesterol levels. So this finding is usually discovered during a routine cholesterol screening.
An individual is considered to have hypolipidemia if his total cholesterol levels are below 120 mg/dL and LDL cholesterol levels (also known as your “bad” cholesterol) below 50 mg/dL without the use of cholesterol-lowering medication. There are many causes of low cholesterol levels, including certain heath conditions and inherited conditions.
I'm not medically qualified to give advice so please contact your Nut (or even Sue Manchester, Dietician & Rhode Island Hospital (401) 640-2999.

Hi Kerry,
The Rhode Island Hospital meeting is the first Wednesday each month. I didn't have my surgery through Dr. Ryder so I can't tell you who she recommends for the Psych Eval. However, the local 'bariatric friendly' nutritionists include:

There are several support groups in RI. It doesn't matter where you have your surgery or who performed them. We are welcome at any of them.

How low is to low?? and also, what can I do to increase it back to the "normal" range, aside from eating eggs until I turn into one!
I havent asked Dr Roye about it b/c I had to cancel my appointment and wont be able to reschedule until sometime after January. If anyone can give me any info, i'd appreciate it. Thanks
