Track every bite you eat, cut out any carbs you have let sneak back in.
Somewhere on the board it a meat reset plan that may be a good starting place.

What really worked for me long term - even now 8 years post op is "calories rotation". Making sure I confuse the body. I would average 800-1000 per day during the week but I would have days that I would have 1500 calories followed by days with much lower calories intake. And I make sure I don't eat the same food every day. Change the routine. Less fat one day - more fat the other. I ditched all the fake foods - protein bars - and ate "real" food...Too many protein - in easy to digest and absorb protein shakes - my body can turn into sugars... So I made sure I did not eat more than 30gr of complex proteins _with fat, per a meal... I added healthy fat - like avocado, olives, even bacon fat (too much bacon makes me nauseated- unfortunately) to my diet.
Because of my personal health condition - adrenal issues - I need to add some carbs to every meal to make sure my BS is stable...I found the 85% dark chocolate to be a blessing - as dessert. But even a few berries (few - like 5-6 strawberries) also did a great job.
I have days that I am hungry and eat a lot (relatively) and days like yesterday that I had to work hard to get my 60gr of proteins in me. At the end - my weekly average should be 80 gr per day... And that 8 years post op.
I typically don't eat grains, starchy veggies, or dairy... I lost my regain and I maintain my current weight without really counting calories...I weight every day when I am home...and adjust my eating based on that. When I am low on the scale (below my goal) I eat as much as my pouch let me...when I am in a middle of my range - I am very careful..when I am at the top of my range - I really portion my food...
