5year documentation

Hi all,
My insurance say they need 5year documentation of obesity. Does anyone know what that really means? My weight has definately been up and down and in the obesity range for more than 5 years but I dont think it was officially documented in my medical record. Right now I weigh 235 and I am only 5'4ish The only one I remember was when I was pregnant with my 2nd child and the doctor said he couldn't hear my daughter's heartbeat at 14weeks. I was scared to death .... it turned out fine after I did a real ultrasound and I remember he wrote it down that he couldnt hear it due to obese stomach. Does something like that count? I dont know. Any thoughts from anyone whose insurance company requires the same thing? I appreciate your responses in advanc. Thank you!

My doctor had my weight history based on my annual physicals and that worked for me.  Good Luck, I know insurance companies can be a real pain.

It's your dr's record of your weight over the past 5 years.

 Call your insurance company and ask for the bariatric dept. if there is one. Ask them to send you specifics so you can provide to necessary documentation. Also, try to remember back all those different diets you must have tried with the dates, type of diet, # of pounds lost. Records from any doctor visits, they have to weight you at each and every doctor, so there must be records from the past 5 years somewhere. Good luck to you.

Insurance requires records from physicians.  I had to use ob/gyn for a few years because I did not go to pcp. Remember each doctor appt for the last 5 years and call to request medical record with weight listed. That will work!

I started at 245 and 5'4" and ended up being selfpay.  My insurance wanted me at that high of a weight for 5 years.  The fact that I yo-yo'd my whole life screwed me.  But I would pay for it a hundred times over.  It has given me my life back.

My ob/gyn gave my 5-yr weight history. I don't really know if I was technically obese every time I went in but his 5-yr history worked for me.

That makes so much sense. Thank you all for your widom. I really appreciate it. My weight has flunctuated in the past 5years and I had my daughter 4 years ago too so that definately helped balloon me up. This is such an exciting yet confusing journey and to have you all - OH is a great thing. Thank you for making it a lot easier!
