RNY and Adderall

 Before my bypass on Nov. 11th. I was taking Adderall ER. After my surgery my doctor changed me from ER to the regular Adderall. This is not working as well as the ER. I am only 10 weeks out from my surgery, so I cannot take the full ER capsule. From what I read on Web MD I think I can open the capsule and take the granules in yogurt or pudding. I take my nexium this way and it has worked well. Does anyone else have any issues with their Adderall after their bypass? My doctor was concerned about absorption of the ER post bypass. It seems if Nexium works OK this way, Adderall would do the same.

I stopped all ER meds post op. My psychiatrist said that the ER doesn't absorb right because the RNY, and because there's no acids in the pouch to break down the levels of releases. She has a lot of post op RNYers b/c she works in the same hospital group as my surgeon.
I used to take Concerta, but can't take that anymore b/c it's basically extended release Ritalin.  Instead, I take the tablets at intervals. For example, if I took a 30mg extended/controlled release, I take two 20mg tablets at regular intervals (increased total dosage due to malabsorption purposes). Regular intervals being the timeframe you need/expect the previous medication to work. Also, meds I expect that won't absorb properly, I take with a 2 tbsp orange juice to "make up" for the acids.

From my understanding (and a discussion on here about a year ago), the Adderall XR is different than other extended release medications. Most extended release medications are activated by exposure to stomach acid, which is why we have problems with them. I think the Adderall works in the small intestine, or something like that.

I've taken the Adderall XR both before and after surgery (without opening the capsules or anything like that) and have not noticed any difference in effect.

do you take the capsule whole or do you open it up and take just the granules? My surgeon is concerned about taking it whole because of the gelatin capsule getting stuck.

I went back on it a few months out from surgery, so I've always just taken it whole. But I've taken other capsules in a similar capsule since much earlier out than that. and never had anything get stuck. The size is more of an issue than the coating, I believe.

I took gel capsules from day 1 (painkillers) and have not had any problems with meds in gel capsules that have to be swallowed whole - like my Cymbalta Antidepressants - be careful about removing the gel capsule and taking the content ( wheter powder or granules) out of the capsules - some have to be swallowed whole - like Cymbalta.

I was looking into the Adderall ER. I saw some posts about Cymbalta. I have been on Cymbalta for years and I spoke with my provider and she instructed me to just open the capsules which I have been doing for a month now with no side effects at all. It seems to work same as it did before. I have takens small gel caps though, 2 of them, since surgery and they did not get stuck. Thanks to all of you for your advice!

Reg Adderall here. I just take it in 2 doses.
