I actually thougth I was going to get him (my Ins only has two Bariatric Surgeons they use - Dr Paya and Dr. Quilicci) so I went for my consult BEFORE my approval came thru.   I honestly don't recall his requirements.   I know he gave me the list but I tossed it the next day when my referral came thru for Dr. Q.    Sorry I can't help.   He seemed to be a nice guy but that is all I can say.

Dr. Q's office is a machine.   Well oiled most of the time but not a lot of personality.  I think as a whole they do an excellent job and I'm completely happy with my surgery, the outcome and my weight loss.

I refer more on my own PCP and my lupus doc than on Dr. Q's office and I will not spend any time with the nutritionist there as I think she knows not how to counsel people but is a regurgitation of what the doctors tell her to say.

I *love* Dr. Tovar's bedside manner.  He is easy on the eyes and has a sense of humor.  Dr. McVay is kind of uptight but OK.   Dr. Q is just toooo damn busy when there are no complications to even talk to you much besides his own 'Answer these three questions for me'

He is a SUPER SWEETHEART and his bedside manner is awesome.  He is out of network for me now.

I also ended up going with Dr. Quilici. The only thing I remember about Dr. Paya is that he only requires a pre-op liquid diet if your BMI is over a certain point. I think it's 47 or 48.
