Sleeve size for non DSer???

My mom recently consulted with Dr. K to get the VSG. She is a chron's patient and can not have her intestines messed with.

Keshishian says that he prefers to give a 4 oz gastric reduction to avoid the issues of strictures and persistent nausea and reflux seen in those with tight sleeves. He mentioned that Baltasar did a study recently showing that a bigger stomach does not have a significantly different outcome than those with tight sleeves.

This just doesn't make any sense to me. 4 oz sounds way too big, and I would be afraid that she would regain a lot with that size stomach. What are your thoughts on this?

I hate to say it,  but eventually she's going to regain.  The sleeve will stretch, and without the malabsorption,  the weight will come back.

I eat at least as much as I did pre-op,  and have been for several years already.  I'm 11+ years post-op but my capacity returned to pre-op capacity a long time ago.

You have the DS?  Wasn't it standard to make the DS sleeves fairly large 11+ years ago?  And did they use Bougies then?  Do you know that example of the Bougies that goes around - it shows a Bic pen, Sharpie etc to show Bougie size?  It really is much difference - right?

Can you give me an example of what you can eat?  Is bread an issue at all?

Yes I have a BPD-DS and a sleeve gastrectomy.  My sleeve was small - my capacity was very limited for a long time.  But eventually......not.

I can eat a huge meal,  like a big steak and a baked potato and a vegetable and wine and dessert and appetizer and salad. 

Yes of course bread is an issue,  like simple carbs are for almost every DSer.  Not that I don't eat it - but it has consequences.

Wow, this makes me think - what's the sense of  having surgery at all if I will end up in the same boat a couple of years down the road.  It has happened to me with the Lap Band and it sucks more than I feel I can deal with at times.

When I asked if you had issues with bread, I did not mean what it does to our system DS or not, just whether or not it was difficult to eat.  Bread in any form (I eat the most complex types) is the biggest culprit of my inability to lose or keep weight off.

On the other hand, I look at Ms.Batt at 7yrs post-op who still has some level of restriction.  I can only hope that if I go through with my Sleeve that I will be like her.

 A lot of us don't have near the restriction that we had early post-op, that's where the malabsorption comes into play. I'd be screwed without my malabsorption. I can eat a footlong flatbread from Subway and chips and a drink. Early post-op I was lucky to get down a scrambled egg.

Wow, that is crazy!  I wouldn't even dare eat that flatbread now and I can.  I skip the chips and the drink too.  Don't get me wrong, even when my band was still in and working I never kept it tight and so I'd order a  a honey oat scooped out as much as possible on both sides toasted with turkey, swiss and  bunches of veggies.  I'd eat 1/8 th of the footlong bread.  I am a creature of habit and still do the same today.  Yeah, I could order the salad but I'd miss that little piece of bread and I guess the entire process - the idea of a "huge sandwich"

You know, I am not a DSer and carbs bloat the hell out of me now.  No doubt, I was born round and hungry- my body is back to it's set point.  It is a fight that I am willing to fight.  I feel good about myself when I eat good foods but less.  I feel strong and proud when I exercise . . . especially when I see the results.  Right now, I spin on my bike and wake up fat day after day. 

I think the DS is grand and I have thought about it through the many years.  I am however convinced that I need only some hunger control and restriction.  There are also too many strange things that happen to my body to mess with malabsorbtion.

Just be aware that it is entirely possible that the restriction will diminish or disappear with time.   That's where the malabsorption does its job.

Restriction plays no role in my weight now - none whatsoever - I have no restriction.  It is the malabsorption that works for me.  I have a 75cm cc, fwiw.

If you are eating relatively healthy and exercising now and still waking up fat - you NEED the mal-absorption of the DS.

If you have dieted and exercised pretty much your whole life - you need the mal-absorption of the DS.

I think you are deluded if think that all you need is restriction at this point, especially since you have already had one WLS fail you.  Why set yourself up for another potential failure?  That makes no sense.
