STD Coverage and forms? Who, what, when?
I just dealt with this today, my insurance - Blue Cross assured me my GP could complete the forms, as most likely that is who will be managing the return to work, post op appts. etc. so I faxed them, she completed what she could but said the surgeon has to fill out the recovery part. so I am charged 75.00 for that...then I called the surgeons office and they said the forms can be left with the surgeon while in the hospital and picked up on the day of discharge. of course another charge. I am asking the insurance co to pay the first 75.00 as they gave incorrect info. and I am thinking of taping the insurance form to my tummy sure to be noticed by the surgeon..I am sure he wants to get paid...so do I ! I faxed all other forms today, 4 days before surgery so the Insurance co. can at least start the review and I will fax the surgeon form on discharge day...hopefully it will all work seamlessly lol

For me, my HR dept gave me the form they needed completed to submit to SunLife. I had to drop it off to my surgeons office (plus pay a $40 administrative fee) and the admin assistant mailed and emailed me the form back within the week. It was a bit inconvenient because they wouldn't fill it out until after my surgery, so I had to stop by the office once I was released from the hospital.

I work for a massive Canadian company and the STD process is a handful! a third party compamy (Morneau Sheppell) reviews and decides if you ar approved or not.. my manager has to request the process then a triage person called me to get information about the surgery. the reason morneau shepell does it is because I do not have to disclose what kind of surgery I am having to my company,, I had to get these forms filled out on the DAY OF SURGERY by my surgeon.. and submit them within one week... the forms were filled out half assed by his secretary and then signed by the doctor.. I was amazingly approved, though.. and am officially paid full pay while I am off.. what a releif!
ETA: almost all surgeries are elective. as long as it is covered by OHIP you should have no problems..
