Panniculectomy denied - appeal letter guidelines?

Hi all - I recently consulted with a plastic surgeon for a lower body lift. We submitted a preapproval claim to my insurance company (for them to cover the panniculectomy portion) and it was denied. I'm going back to my plastic surgeon's office tomorrow to have more pictures taken for an appeal, and the nurse said that it would help a lot if I had a letter for them to send in with the appeal detailing how my panniculus is impacting my life (rashes, just getting in the way, etc). I know what I want to say, but I am having a really hard time getting the letter started! Are there any guideline letters anywhere, or do you have any words of wisdom on getting the letter started? Help! :)

I am in the process of appeal myself.  What is the reason they gave you for denial?   GET ALL the info you have!  A letter from your PCP, all the medicines you have tried and used to heal and prevent and the dates in which rashes/infections have occurred along with pictures if you have them or doctor visit to prove.   It's a pain and it'll be a fight but you can do it!!! HUGS!

Start with the reason for the denial.  Then give them chapter and verse from their guidelines why you believe you meet the criteria.  Address each issue.  In the end it will be getting them to see how you meet their criteria for approval that is going to be what wins the appeal.

Howdy neighbor! Just looking at your frame on a picture in your profile blog, I thinks it's going to be unlikely (as I'm imaginging your pannus) that  you will meet eligibility criteria insurers have here in Alabama.  Generally, if you don't have a large residual pannus hanging to mid-thigh you aren't going to get approved by BCBS is my impression from going thru this process routinely. They scrutinize these claims closely.
