If you're confused, call his office and ask about it. I don't really understand why he would need a food journal for a psych eval.

I often try not to step on the toes of other professionals (i.e. nutritionists), but I'd encourage you to call him for both an earlier appointment and to ask for clarification on what specific behaviors he's concerned about. Maybe a month is sufficient time to change behaviors, if you had clear in your mind what needed to change, but it doesn't sound like they'd been made very clear to you. I'd be concerned about the food journal part - he's not an RD, right? What people are eating is not my place, as a therapist, to address but if we're working on the emotions associated with eating, yes - that info is pertinent.
