My insurance is BC/BS Federal (wife's employer) and they covered WLS provided you have a BMI of 40 or higher, OR a BMI of 35 with 2 or more co-morbidities (diabetes, sleep apnea, congestive heart failure, severe edema, etc.). Although we have the BC/BS Federal, since my residence is in Alabama, I did have to communicate with BC/BS Alabama and found they have the same requirements. Then, since I opted to have Dr. Richards at Vanderbilt's Center of Excellence (BC/BS recommends using a Center of Excellence) in Nashville do my surgery, my case was actually handled by BC/BS Tennessee out of Chattanooga. It is my understanding that the requirements are the same for all of the BC/BS Major policies - but I am not 100% positive on that.
