Need help with appeal please

I've found the one appeal letter on OH that is a sample. Would anyone else be willing to share a copy of their successful appeal (with the personal info removed of course)? I'd like to see more than one sample to get my husband's appeal going asap.

Our insurance is with UHC PPO (and covers WLS)  and we have to show a five year history of morbid obesity.

3 years ago my husband lost 100 lbs due to pain pill addiction. His BMI dropped to 39.6 and so  UHC has denied his surgery due to being under the BMI 40.  Have any of you encountered this type of denial?

 Do you mind telling me if you successfully appealed something similar?

 I'll take any hints, recommendations, samples etc. that anyone can give me.
