Aug 2008:
6th - WLS seminar at Baylor Hospital
11th - Insurance discussion with Baylor
12th - 1st - PCP appt to begin supervised diet & behavior modification
18th - Appt. with Head of Bariatric Surgeons at Baylor
19th - Psychological Evaluation
Sep 2008:
12th - 2nd - PCP appt. *lost 6lbs*.
12th - Spoke to Insur ofc @ Baylor & they will be sending all insurance information today including letters from Surgeon, PCP and Psychologist.
30th - Insurance rcvd file and in their system as of today.
Oct 2008:
9th - GYN appt today - will need (hopefully) minor surgery 1st of year
10th - 3rd - PCP appt. *lost 5lbs*.
15th - Baylor insur ofc. called medical insur company and it hadn't been approved yet
23rd - last GYN testing before surgery
24th - Received preliminary approval letter from insurance company
27th - Spoke to Baylor Insurance and explained will now need to wait until (guessing)
end of January for WLS due to GYN surgery few weeks before
Dec 2008:
10th - GYN appt for test results & to schedule surgery
Jan 2009:
6th - GYN did hysteroscopy and D&C today *down 7lbs*
16th - Biopsy report clean - no cancer
21st - Cardiac Clearance test scheduled for WLS - Cleared for surgery today!!
28th - Consult. w/ Doctor to schedule EGD
29th - Surgery date 16th Feb - *Canceled*
30th - UHC informed Baylor Hosp WLS no longer covered by my company
30th - Baylor Insur coordinator will resubmit to UHC for approval
Feb 2009:
2nd - Sked NUT appt - *Canceled*
3rd - Sked EGD appt - *Canceled*
11th - Baylor resubmitted request
23rd - UHC wants additional information:
1.) Thyroid check (been on medication for 9years)
2.) 3 pre-op NUT counseling
3.) attend support group
4.) Exercise Counseling w/Physical Therapy
5.) Psyc Eval (already done in 2008)
Mar 23rd thru March 27th - Completed HOOP JUMPING for UHC
*** 27th MAR - SURGERY APPROVED & SCHEDULED for 27APR ***
