Cigna insurance - Does anyone know how good Cigna PPO insurance is and also how hard is it to get approved for surgery with them? Did they require anything crazy?

Insurance - Can someone tell me if you have any options when your insurance policy has an exclusion for surgery? I really need to have the surgery, but our family lives from paycheck to payche...
