Carbs
Net carbs means total carbs minus carbs from fibre. So, if you eat something with 10 total carbs, but 3 grams of fibre, it would have 7 net carbs.
Personally I have always counted all carbs and lose best when carbs are low. The idea that carbs from fibre are not absorbed is not totally true; your body does absorb some calories from fibre. Lyle Macdonald talks about this in his books. For example, if you eat 100 calories of fibre, your body does absorb about 40 of that (now that's for a non-RNY person, I don't know exactly how our malabsorption affects this).
I don't count carbs at all.
I do eat a very low carb diet, but I do eat fruits and veggies, the occasional sweet potato or tortilla. I also have a home made muffin (made by me so I know what is in them) every once in a while, noodle soup (with very few noodles). I rarely eat bread (it still sits funny 8 months out), potatoes or rice. I never eat junk, no chips, ice cream, chocolate etc. THAT is amazing to me!
