I spoke to Dr. Mueller's assistant today and she has received everything she needs to write up my request to the insurance company.  As soon as she has completed her work, she will submit the request and send me an email.  It should take 7 - 10 days to hear back.  Once they approve, she will call me to schedule the surgery - woohoo!
