well heres what I know I was finally aprroved in sept for lap band after three denials and 16 months pre req for Bcbs ppo was 12 months documented weight loss failure and as my luck would have it it was changed in sept (better for you maybe) to 6 months documented weightloss failure but hey also changed the BMI requirements to a higher # best advise stay on top of it and fight it al the way found customer service to be very nice and helpfull good luck its worth the fight
I have BCBS as well and would agree with the other responses. I was turned downed my first time as well. But they changed their policies back in Sept. You have to have a least 6 months of documentation working with a Nuturitionist and a failure to lose weight. After working with the insurance company for 2 years, I finally got approved. My surgery date is February 6.
So, hang in there, do your research and keep up the fight. :-)
I have BCBS PPO through Carpenters. I was approved the first time, but it did take 4 months. I had to go through a health coach. We had early morning conversations every 2 weeks to discuss what I was doing and again go over all my questions, thoughts, anticipations, etc. Then had my psyc review. I passed! Then it went before the review board who actually approved me. They then sent a letter to BCBS (Carpenters) and then they sent me a letter saying that I was approved. I got a surgery date of 3/7/07. I just have to do my pre-op testing. Good luck on your approval. It's the best feeling!
