Hi dietary qustionaire for BC/BS if IL

The dietary questionaire list 25 non surgical diets that I may have been on. I have only been on five of them. Will this hurt my chance for wls? I think I have met the medical criteria for wls has anybody filled this out before. Please help. :- (

I am not sure if you have BC/BS PPO, HMO, or what, but I can tell you about the PPO. They ask you those questions because their rule for the PPO is you must have had 6 months of Supervied Diet with behavior modification, and exercise. If you have been on WW or Jenny Craig you would have some sort of proof of weigh ins etc. They would have to be within the last 2 years or they won't count towards the 6 months. Also they must be consecutive. If you missed one weigh in, in a month you have to start all over. So say you go in May but miss a weigh in during June, then go in July. July is now the First month instead of May. They are just trying to see what you have tried, and then you must provide the proof. Even then they can give you a hard time, although I am not to that point. I am trying to stay one step ahead and started a Medically supervised 1200 Cal diet before the first insurance answer can come. I do meet all the rest of the Criteria, I have a BMI over 40 so I don't really need to show any comorbidities, although I do have sleep apnea and when I weigh more my BP goes up and I was already on medicine once. I lost some weight and got off of it, but the weight is slowly creeping back. You must meet the co- morbidities if your BMI is 35 or less. Also the more comorbidities you have the better your chances are. You have to see the Psychologist, Cardiologist, and Pulmonologist. Which I have done already and asked my PCP for the 5 years of chart records to show I was morbidly obese during the past 5 years. Once I finish the next 5 months up I will be all done and ready to see what the insurance says. I hope this helps, you can log onto BC/BS of IL and see what they say for GBS.

Thanks for the prompt answer, I have an HMO and several medical problems related to my weight my BMI is 49 and I have sleep apnea,diabtes, thyroid and high cholesterol and I am only four feet 9 inches tall so hopefully i will not be turned downed. Thanks so much!!!
