Just curious...I have bcbs ppo as well, my pcp sent a general letter stating i have been a patient since 01 and m.o. since 03, recommends the surgery, said that we have "tried counseling, dietary therapy, and weight loss meds (meridia) for past 2 years with no benefit" and that it is medically beneficial for the surgery. has enclosed copies of his notes. am i to assume it has to be specific, actual copies of my med charts or would you think his letter is good enough with the notes? i am in the process of all the pre-appts. i have my surgery consult on the 23rd. if you have anymore info that i should know please share!! thank you so much!!

Hi Maria, What BCBS is looking for is a five year history of obesity. You wll need to get records from various doctors showing your weight in that time frame. The monthly requirement is for the the supervised diet history with your doctor. The required time is now 6 months and not 12 when I began my journey. Keep in mind that you cannot miss a month or even be off by days in your visits. You will need to have your doctor write detailed notes about your weight plan as well as excercise. They also look for evidence of diet drugs unless your doctor can provide a valid reason for you not being on them. Hope this helps. Mary
