APPEAL LETTERS

 Hi my name is Jeaniene and I am looking for anyone who might have an appeal letter to share. I need some guidance in writing one to my Insurance-Humana PPO//Michael Reese Hospital. I was denied due to an exclusion. Gary Viscio said it might be hard to beat but to try anyway. So I need a letter to HR and the Insurance Co. But  I don't know where to begin  Any help would be greatly appreciated. I'm trying not to get depressed over this minor setbac****il I've exhausted all my resources. Thanks in advance.
