Last 6 month appointment today!!!-any advice?

Hey friends!!! I went to my last pcp(6month app.) today!!!! yeah! my pcp says he will write my letter today or tomorrow.I have to go to my nutritional eval. and then that's it!!!!! I will drop by to pick up a copy of my file and the letter on Friday. I have a form that one of the oh memebers sent me and I am gonna ompare that to my visits and the doctor said to bring the form by next wek and that he will sign off o them.The forms basically have the technical terms on them like "nutritional therapy" and "exercise therapy" . all those specifics that they want to see. i dont's want any problems and I don't want to be denied based on my 6 month diet.I have a BMI of 47 as of today,I have gerd,joint pain,mild sleep apnea,high cholest.,and stree incontinence.Is there anyhing else that can be wrong? well any wasy I have BCBS of IL(ppo). so do u have any advice for me? should I write a letter to insurance as well? Is there anything else u thik i should do? any advice is greatly appreciated.

Did u get your psych eval also?? I had my surgeon write a letter as well. I know that helped

Yes I had my psych eval. already. My surgeon is going to send in a letter as well. thanks for your response!

It was not recommended to me by anyone however, I chose to write a letter to the case worker that was reviewing my case.  I really worked it up and kinda did a "poor me, pitiful me" letter begging for a second chance at life.

Don't really know if it helped but I was approved on the first try.
