John, with most insurance companies you should be able to go to their web site and find doctors that are in network for you. There are a lot of WLS doctors in the Chicago area. I would check with your insurance first for the list.
As a suggestion try my doctor, Dr J Rosen, 630 261 9393. He is in Downers Grove. He has a well rounded program with pre op care before, support groups, NUT in office, exercise physiologist in office and great aftercare seeing all these same professionals. The physc doc is off location but you still will see one at another office.

i had Unicare HMO last year and I work for the state. Unicare's policies were strict, so I switched companies. I'm not sure if you are able to do that, it's a consideration.
Have you checked on this website the doctors in your area and the Chgo area, the have a list of insurance companies that they have been approved through.
Go to the "look for a surgeon, then click on the individual surgeon" and then insurance. Hope this helps.
