Insurance Question

Hi Everyone -

My name is Jennifer and I'm new to his board.  I would love to meet other folks and talk about our experiences.  I am the mother of 3 beautiful kids ages range from 12-4.

I am in the final stages of preparing for surgery and have met all of the requirements except this final one and that is putting together a Behavior Modification Journal.  I was told this should mirror my time spent at Jenny Craig.  The problem is I'm not really sure how to do this.  Does anyone have a sample?

I am going through De Paul Hospital and the surgeon is Dr. Scott.  He is in with Dr. Dela Torre (if I'm spelling that correctly).

Any information anyone could contribute would be greatly appreciated!

Hi Jennifer and welcome. I did not have to do anything like that but what I think that they are asking for is a time line of your time spent while trying to modify your behavior. I hope that makes sense. Basically I think what they want to see is what the steps are that you have been taking to change your behavior now and for when you are PO. They are not looking for perfection but at least honest efforts to show that you are learning new lifestyle habits and that you will be able put them into practice when you are PO.
