I am glad that everything is falling into place. A word of advice on the supervised diet. Take it as you want, but I had to do mine twice because my doctor did not document EVERYTHING the first time around. My insurance company required a visit every calendar month, and at each visit, my chart had to reflect my current weight, the diet plan I was following, any diet medications I had tried, and the exercise attempts that I had made. I ended up creating a chart for the second go around so that my doctor could attach it to my chart and easily fill it out and make sure all the required info was there each month.
Most doctors will NOT go back and add to your chart if something is left off. I found out the hard way.

I second what Nancy said. My PC Dr. was very supportive and worked very cooperatively with me, but when submitted to BCBS, they didn't "like" the documentation. I started again with my wt. loss Dr's office (BC changed the "rules" and said now the wt. loss dr COULD do the documentation) and it went through. If your ins will let you wt. loss dr. do the documentation, I think you stand a better chance, since they know what the ins co is looking for.
