referral letter

For Illinois medicaid do I need a referal letter from my primary care provider? IF so is there a draft letter on the site? When I call to schedule my psych eval what do I say? What does the letter from the psych need to say? Shelli

Hi! Yes, for medicaid you do need a referral letter. I'm not sure if there is a draft letter on the site. I've never seen one. On my profile, I have what my referral letter said from my pcp and a copy of my psych eval. When calling to schedule the psych eval, I explained that I was interested in having wls and that I needed a psych eval. I was wondering if they dealt with that kind of thing.
