All plans are different - have her call the customer service number on the back of the card and they should be able to give her an explanation of benefits that states what she has to have. I have BCBS and need 5 years of weight history from my PCP as well as a 12 month supervised diet under a physician that included dietary and physical therapy. Some insurance companies do not require anything - just a letter of medical nescessity from the doctor. Good luck!!

Hi Maryann. I am from Illinois and I have medicare and medicaid. You know, every insurance is different, but a lot of people on medicaid are approved even within one month. I am sure that your Mom won't have to be supervised that long. If she can write down a detailed history of diet procedures she has been trying and using in the past, that should be enough, especially if they are supervised by her doctor, a surgeon, dietician, any fitness place, any diet place (weigh****chers, nutrisystem, etc.). Anyone who is overweight has done "something!" If by chance she doesn't have enough information about her diet history, the most she would be asked to be supervised would probably be six months and I think that would be stretching it!! Hope I helped!! Barb
