Need some Bariatric Registry help....

VSG on 01/25/12

My mom is trying to get a referral to the bariatric registry but even though her doctor agreed to the referral today.....they don't have online access at their office (what the heck?!? get with the times..lol) so the doctor suggested my mom figure out how to do it without using the internet. What does my mom have to do here?

Her doctor has to call the registry and register his office and they will send him a pack of referral forms.

My referring physician doesn’t have internet, what do I do now? Doctors without internet can register over the phone by calling 905-522-1155 x.35346 and receive the new referral form and instructions via Canada Post Good luck

she can print off the referral form and they just have to fill it out and fax it to them that is what my doctor did.

My doc called them and they sent over a package of forms. Easy peasy. My doc isnt online either.

I am absolutely shocked my mom wants to do this! She has Chrones Disease and is likely going to be able to get a VSG like me which I think is why she decided to do it!
