Need some Bariatric Registry help....

My dr. sent in a fax. If your moms dr goes this route tell your mom to call and make sure they got the fax.  My first one was lost in the shuffle and had to be sent again.

 I would better dollars to Optifast packets that the Dr is putting up hurdles. It is highly likely your physician bills OHIP online. I can't imagine ANY physician willing to wait for paper billing to be completed and then get paid.
