The Sleeve and higher weight...opinions?

I honestly think that you would probably do really well with a sleeve. It seems like the young folks have an even greater chance at losing ALL the excess weight too even with a higher BMI. Your age is definitely an advantage :-). The DS does definitely not guarantee losing all the excess weight and keeping it off either. I know a lady who only lost 125 of the 250 she needed to lose even with a duodenal switch with a short CC. You can out eat any surgery if you want to. It's the plain truth.
I am 29 and was 28 when I was sleeved - 452 pounds and if you ask my surgeon he want me to reach 190 pounds!
I laughed and said 225 is my goal - a solid 225! My surgery was Nine Months ago on Friday and I am down 203 pounds!
Because of your age i suggest the sleeve, but ultimately either will work if you let it. This is only a tool and you have to have a solid mental approach!

I had exactly 190 lbs to lose. I am 33. Not yet two months out and over 40 lbs down from my highest weight. I knew someone who had the bypass and saw them throw up every day, lose their hair, and turn ashy gray only to gain half the weight back in the next ten years. Needless to say, it was a no brainer for me. Not everyone loses 100 lbs in 6 months with the sleeve but it's a great tool!
Most people can lose large amounts of weight with ANY form of WLS. The real trick is keeping it off, long-term. Those are the stats you need to be looking at. There's no question but that the DS has the very best long-term, maintained weight-loss stats. But yes, it does come with a price---rigorous daily vitamin supplementation. If you can't or won't do that, then you absolutely should NOT get a DS.
I'm pre-op (one week to go) plus I'm a lightweight, so my personal experiences might not be super relevant. However, 300 Pounds Down is a blog written by a woman who started at 400+ pounds and had a vertical sleeve gastrectomy. She's pretty frank about her struggles and about the hard work she still has to put in two-ish years out. It's a very positive blog, though, and helped me feel secure in my decision to pursue a vsg. Might be a valuable read?
IIRC the author, Holly, has lost about 200 pounds since surgery and plans to lose more. I'm not sure what her final goal is, or if she's holding herself to a specific number.

Well, RnY and VSG have similar results so if VSG isn't going to be enough, RnY probably isn't either...
I've been on this forum for years and what I see is that people who diet well do well with the VSG even when they are at a higher weight. There are people here who have lost way more than 190 pounds. BUT they always lost weight when they dieted. Their problem was that they couldn't keep it off, not that they couldn't lose it to begin with.
The ones who have trouble losing weight with diets tend to lose somewhere between 70 and 125 pounds and then stop. A lot of them go on to get the DS and the ones that don't have a lot of frustration trying to maintain and trying to lose more.
So... be honest with yourself and how you diet and that will give you your answer IMO.
Oh and one nice thing about the VSG -- you can get it and then see how you do. If you don't need it, you don't have to get the second part that makes it a DS. But, if you do decide to go that route, you need to do some stuff upfront to make the insurance work out for you and pay for both surgeries.
