Methyl DOES absorb better -- no doubt about it.

However not everyone NEEDS methyl.  Some can be fine on cyan.  Me, for example.  I have a 1748 on cyan.  It is really dependant on labs -- only your labs will tell.

Keep in mind that b12 can tank anywhere between 6 months and 5 years out.  So you need to watch your labs very carefully.  If you are supplementing with high-dose cyan and it's not working, then yes you absolutely must switch to methyl.  If you want to bypass this step, then by all means switch to methyl.  But not everyone must switch to methyl -- it's not like calcium citrate / carbonate in RNY guts.



ETA:  I saw a study about injections vs sublinguls.  Supposedly there is no difference in absorption, btw.  I'll see if I can dig up that study (said as I'm supposed to be studying riboflavin...)

Andrea, that's true for injectables.  They bypass the need for intrinsic factor, and therefore absorption is comparable in both forms.

It's true for all forms.

I did misspeak.  I have a 1721 on my current labs.




Some people do absorb the cyan version of b12.  It's just the way it is.  And we can't say it's due to a build-up and I'll tank cause I was at 200 something when pregnant with Kaitlin (that's when I tanked...  they threatened shots then and I held off).

Yes, methyl absorbs better.  We know this.  But cyan does absorb in some people -- perhaps even most.

Hey, I was as shocked reading it.  I only convey the study -- I did not conduct it! ;-)

And it was in regular gutted folks with full IF -- which we don't have.  So take that into account.


I need to put this in my signature line -- your labs should always be your guide.

I also wonder if there are studies to back this claim up.  If there is, there are an awfully lot of nutritionists and bariatric surgeons who are advising their patients otherwise.

Unfortunately many surgeons or WLS centers that are supposed to be "the best" in the field give really bad vitamin advice.  I don't know why.  All I can say is that they may be great at cutting and sewing, but that doesn't mean they are great at vitamins.

That said, the truth is that many people do just fine with cyanocobalamin.  Everybody absorbs methylcobalamin better, but many people absorb cyanocobalamin well enough.  So the product they are selling will work well for many RNY folks, most of them, in fact.

Take it from someone who learned the hard way...9 months post op, my feet went numb, I had terrible brain fog and vertigo, and tingling in my back, face and mouth.  I thought I had MS.  I could write a thesis on B12.  MethylB12 is REALLY superior to cyano.  I could go into the chemical reactions and cite the reason why...but just trust me that you really should be taking the methyl form.  The best on the market are made by Jarrow.  I take 4 (yes4) 5000 mcg sublinguals per day.  They re pretty big but taste absolutely delicious.  They dissolve pretty slowly, which is what you want.  You do NOT want to get deficient in this vitamin.  I also do shots 2 times per week. Once you start feeling the symptoms of B12 deficiency, your stores are so profoundly depleted that it takes a long time to replenish them.  I hope i get the feeling back in my feet.  It could take months...and could possibly be permanent.

Having said all this...my RNY is the BEST thing I've ver done.  Good luck everyone.

I wouldn’t advice on Bariatric Advantage vitamins. They are a complete waste of money. They made me nauseous each time I took them. Worst of all the bottles did not have complete information on the ingredients. There was some info missing. I have been taking my vitamin b12 sublingual from vita4lifein a serving size of 1 vitamin b12 sublingual in the form of methylcobalamin with 1000mcg for six years now. My labs have been perfect and have had no nausea.
