Hi, I'm pre-op until Tuesday, and have been on Celebrex. I have been told that it will have to be Tynenol - arthritis, now and afterwards. The pain has been awful because Tylenol anything doesn't help me. So if reduced weight is a big element, then maybe it will be okay. Thanks for tips on other meds. I taught Arthritis Pain Management for Stanford Univ. for 3 years, but pain is pain! Judy

Topic: Is Gout considered a non -medical co morbitity?
I have Gout in my left foot and was diagnosed almost a year ago. Most of the time it doesn't hurt but sometime it can feel like a thousand bees are stinging my foot, and it swells up. I didnt get gout until i gain about 40 lbs in two years. I wonder if this will help with my insurance. I hope so.

I have arthristis in my right knee. And post- op 18 months. And the only Anti- inflammatory, I am allowed to take is Nabumetone. But I am only allowed to take it every other day and no longer than 7 days. There was 2 non-steriodal pain killers that I could take also. But I don't remember them off hand.

Topic: RA and RNY Surgery
Hello all, I am so glad I found this message board! I am scheduled for lap RNY in August. I have RA and currently inject with Enbrel, take methotrexate and Celebrex. I am wondering if their are post ops with RA and took similar meds pre surgery. I am just concerned about what to expect. Any info would be greatly appreciated. Lynne

first first diagnosis was gout. However, my rheumatologist told me that women do not generally get gout. If I were you, I would get a second opinion. After nearly three years of going progressively losing my mobility, I finally went to a rheumatologist...he immediately put me on proper medication and I was pretty much without pain and greatly increased mobility within a few days. I would definitely get another opinion.

I am about 5 years out from having my right knee replaced. The pain after surgery was never as bad as the knee pain before. The rehabilitation was lengthy, but prior to my surgery, my knee had frozen and I had to have an abnormal about of rehabilitation...18 months. It is usually only about 12 weeks to 6 months max. Just like with WLs there is a period of time when you ask yourself...what have I done?...during the first year. But that passes and soon, other than setting off alarms at airports, you won't know it is there. It feels very natural...I can even dance....that took a while. I can ride a bicycle, but I went to a tricycle, because you don't want to take a chance of falling. I also notice that I cannot do things that require a lot of pressure ...like pedalling uphill on the tricycle...but really, how often do you have to do that. It'sl ike WLS...if you need it...it is a miracle.

Yep, I've got it, and yes, I've had Lumbar Epidurals. I've had FOUR of them on a regular basis. They do help, but only for a limited amount of time, but it feels good afterward. Mostly because of the big whallop of steriods that is deposited along with the local pain anesthetic. The anesthetic helps in the short term, and about a week later, the steroids kick in......what a major relief. The procedure takes 15 minutes, with an hour recovery at a day surgery center. I get very light IV sedation and it doesn't hurt to have the procedure done. Just be sure you go to an anesthesiologist who does a LOT of these, and be sure you get an MRI done before, so you'll know exactly if it's just degenerative disease, or if there's something else mechanical going wrong with the bone structure of your back. AFTER the epidural, if you're not already doing it, gentle back exercise really does help. I see the trainer at the local hospital fitness center, and do the exercises religiously. It really has helped to strengthen the back muscles, and the GENTLE exercise only takes 5 minutes a day. Start very slowly, I only did one or two reps of each thing for a long long time. If you have time, research the topic of lumbar epidurals on the internet too, there are lots of pain management centers that have great info out there.... Good luck with your quest. This is an excellent pain management pathway that may help prevent you from needing back surgery until after you lose a lot of weight.

Sandra - I am hoping to clear the insurance hurdles sometime by the end of this year 2005 and I too have RA, taking injectible Methotrexate and other immunosuppressive and NSAID drugs. My RA doc and an infectious disease doctor say that switching to the "biologicals" like Enbrel, Remicaid, Humira, is the right thing to do. I only take Prednisone when a flare-up hits......but both say that AFTER surgery, we might need and really require steroids to help the body cope with the trauma. Both say we'll wait till AFTER the surgery to determine that....be sure that you have preop labs for the RA, just to have a baseline. Check your RA and CRP levels for sure, and get a sed rate done. The surgeon's concern was for compromised healing, and I've tried to cut back or get off the arthritis meds because the surgeon's NURSE said to do that. I consulted with the rheummy and he agreed to try, and I had a terrible crisis. The rheumatologist has now said NOT to try to modify the meds until just at surgery, and he will call the surgeon and discuss directly the course needed. Lots of people have major surgery with arthritis drugs and do just fine, we just have to be more vigilant. I also take pain medicine regularly, and this is VERY important to tell the anesthesiologist, since we may require extra pain meds not because of the surgery but because of the RA. Things may hurt a lot because of the underlying disease, but it can be coped with effectively. Best wishes for your "event", and hope this info helps.
