I am in a similar situation. I was told that since my deductible for the year AND my maximum out of pocket for the year is met, AND the doctor I was planning on using is on their approved list (yet not in one of the Centers of Excellence) I can proceed with surgery at my chosen facility. The nearest excellence facility is 4-6 hours away by car. I prefer to be closer to home, know my doctors, and be familiar with the facility. I need a hip replacement but now have learned that I MUST get some weight off first.......Not happy, in pain, and trying to deal.

im curious what the process is for the insurance Paramount Advantage. Does it require a diet pryor to surgery or what

Had RNY 2003 in San Diego, CA. My surgeon no longer practices. In 2003 wt. 281 lost 75# then no more. In Oklahoma 2005 went to see Dr Ronnie Keith in 2006-2007 had endoscopy and was told I had two ulcers and a VERY enlarged stoma. I worked so hard to eat right and walked every day (4-6.5 miles) in 2011 made it to 155 which lasted about 6 months. Had L knee TKA 2011, R hip THA 2012, L hip THA 2013, shattered L femur two surgeries 2015 and 2016, R knee TKA 2017.

I am currently filling out paperwork and it asked me for Insurance Information and Guarantor Information. I know I put down my stepfather's name since he is guarantor but it asked me for the Card Holder information. Should I put down my name or his name? My name is on the card along with my own ID Number and Group Number but I'm still under my stepfather's insurance since I'm under 26 and he gets the insurance through his job but he pays for the insurance. It also asked me for the Card Holder's Employer. I'm unemployed but my stepfather is currently employed. What information do I put down for Insurance Information? Mine or my stepfather's information?
