Please add your name, hospital you will be going to for surgery, and any other info you currently have. If you don't know how to add it within your hospital, just put it on the bottom. I pop into it occasionally and do a sort by Hospital.
You can come back any time and update it. And don't forget to SAVE before you exit. Thanks.
