Insurance Forms!!!

What is up with getting your insurance forms filled out by the surgeon???? My surgery was on Dec 21, 24 & 26th and yes 3 times they had to go in. Just received a call from my insurance co who advised me that they had not received anything yet. I brought my forms when I went in for my first surgery and told them to put it in my f ile but they said it would only get lost and to speak to the surgeon before surgery which I did and he replied to give it to his team tomorrow when they come to check on me. Attempted this but they also said it would get lost in their rounds and to bring it up to the bariatric center myself. Shortly afterwards I received a visit from the nurse Sandra and asked if she could drop it off for me and said no problem......soooooo where are the forms? This whole process has been a nightmare for me....cannot wait to put this all behind me.

 I don't have an answer about the insurance forms...hopefully someone who has already been through this will give you a good answer.  But I did want to say that I'm sorry to hear that you had such a rough time and I hope you heal and recover quickly and can leave the bad memories far behind you.
