Called my insurance before they closed and we don't have it where I work. On another note, you can apply for employment insurance for the time you are off.

I got a note from the surgeons office for the time off, then provided it to my HR dept who forwards it to our folks Shepell. They will contact me. It should be done through your workplace.

I got my family doctor to fill out the forms before I went off as they are also aware I am having the surgery. However, you can also contact the NP at the hospital and they will fill them out for you.

Referral- March 2012, Letter April 19, Orientation TWH- June 6, NP - July 3, Sleep Apena test July 16, Internist and SW - July 17, Nutritional class - July 23, Dietician appt. July 30th, Psych-Social appt - Aug 20th. Follow up with doctor sleep apena Aug. 28th Surgeons appt. - Dec. 14th Dr. Jackson. Surgery date - Feb 12 2013 - VSG

Everywhere is different in terms of employer demands. My employer was explicit in filing out the dr's form AFTER surgery. So since our clinic (guelph) wants you to set up a family dr appointment within 2 weeks of your surgery I have already booked mine and so will take my forms to be filled out by them). In the meantime the company is paying me until the forms arrive and then will switch me to Short Term Disability.
