I have my PATTS appointment tomorrow at 8am and I just looked at my booklet. I only got my patts appt around 2 weeks ago because my surgery was rescheduled to an earlier date. Unfortunately my family doc hasnt had any appointment for me and I realized that there is a form to be filled out by my physician to bring to my patts appointment! I dont have mine filled out :( Can I still go to my patts?! Or do I have to cancel?? :(

Your can still go to your PATTS appointment. Just explain what happened and they will have your doctor fax them the filled out forms. Just make sure you get the forms to your doctor ASAP. Good luck!

We may be meeting sooner then expected! I have my 3 month followup tomorrow morning at 10:30. I also have to have blood work done. So you may still be there when I go to the lab!

Where do you live again? Is it down Kingston way?

I have my pre op appointment next Tues and they told me I can fill out the forms myself.....I and going to Guelph.

The one through Humber has to be filled out by the family doctor. He does BP vitals etc. Answers some questions. Kind of a clearance for us on his end.

I never received the form prior to my PATTs . It just needs to be faxed in by you GP before the surgery.
