medicare documentation for unsuccessful weight loss

HOw long and how do you prove docuementation for unsuccessful weight loss for Medicare Texas?? New to all of this please help!!! Thanks!!

Hi Judy, it usually is a period of 3-6 months. You need to be supervised by a dietician AND a physician. Medicare wants to see clinical documentation. The visits have to be consecutive not at random. Good luck. Mona
