Insurance frustration

Trying to do some detective's work on what the doctors office submitted to insurance and why it was denied.

Find out they are tricky son's B#$ches....
They are saying I am non compliant because I did not go for checkups after a year an half. I didn't have insurance and moved from the family doctor.

How do you make this sound good without being non compliant?

i was denied twice, after that i appelled it and my doctor wrote a very mean letter to the insurance company... they say that i also wasn't compliant with them... i needed proff that i was going to the gym and that i was seeing a nutrionist... well i got them a letter from my personal trainer and the nutrionist along with the letter from my doctors and i got approved... it took months but im going in for surgery on April 6... finally... fight it and they will aprove you... they deny you thinking that you are not going to appeal it but if you do they will approve you....
