Who has Blue cross blue shield of Arkansas?

I want to know long was the approval process and did you have to have a certain BMI for the sleeve?

I don't know of anyone that has been approved for the sleeve in AR using bcbs.
I have AR health advantage (BCBS).  They won't pay.
My doctor said AR BCBS won't consider it.
If you find out anything differently, please let me know.

Health Advantage definitely won'y cover it at all. Not sure about Blue Cross. They used to pay a limited benefit on bpass and lap-band but not sure about the sleeve. The limited benefit was $4500 max after deductible was met.

You should call customer service and ask. You won't know for sure until you do. Your benefits are determined by your company (if covered through work). All carriers cover whatever the customer (your employer) wants covered. If surgery is covered, they can give you all the requirements; i.e. BMI, comorbidities, required nutritionist appointments, etc.
