Hiatus Hernia Complication

Hello everyone, this is my first post. I have been reading the forums on surgical weight loss for some time and I'm pretty excited about the possibilities. My BMI is just over 38, but it's been climbing steadily for many years, despite diets. I'm thinking that VSG is the way to go.  I live in Canada where we have "free" medical care so I'm working with my doctor to see if he will refer me to a specialist that can do the procedure. He seems uncertain and it's sounding like the process will take quite some time. Because I'm almost 59, I feel I need to do something fairly soon. I may just go to Mexico, self pay, and get it done. Unfortunately, I have a condition that may be a problem. I had laparoscopic surgery for a Hiatus Hernia in 1996. I have identified a surgeon, Dr Aceves in Mexicali,  who has experience with VSG and Hiatus Hernia.  It's more expensive since it's listed as revision surgery, but I'm OK with that as long as it's feasible and safe. Has anyone in this forum had VSG surgery AFTER a Hiatus Hernia operation?

No,  but I had hiatel hernia surgery during my sleeve.
how long would you have to wait in canada verses mexico?

Thank you for your response. It seems like you can get it done in Mexico within a few months or less.  I had 1 clinic in Mexico ask if I wanted it done this month!!  That seems too fast since my research indicates a pre op diet is required.  As for Canada, I'm not sure.  A whole lot longer. There are some clinics where the wait times are measured in years. State sponsored health care ensures that demand is always greater than supply.   I'm ready to go anytime. While I have encountered several people who had the VSG and Hiatus Hernia done at the same time, I have yet to encounter any person getting a VSG where Laparoscopic fundoplication for Hiatus Hernia was a pre-existing condition. That's a worry since the fundus is wrapped around the valve as part of the repair. There may not be enough material to allow for stapling and suture around a 36 FR bougie at the top of the stomach. Several clinics backed off as soon as they heard about my situation and several others didn't. That's why I'm looking for someone who has gone through this. Who did the surgery? Was recovery more difficult?  Is weight loss hindered? Etc. Etc.

Can you get endoscopy done in Canada prior to making a decision?  Would give you information regarding the status of your hiatal hernia...did it stay repaired or is there another one, etc.?

I also had one repaired during my sleeve surgery.

Thanks for your suggestion. Based on my previous Hiatus Hernia repair, I get scoped every 3 years or so. My next one is scheduled for about 18 months from now. I don't want to wait that long, so I'm trying to get my GP to either refer me to a surgeon or approve an endoscopy.   I'm hopeful that a surgeon can tell if the valve is loose enough to permit a standard VSG. That may be the case since I've need daily medication to manage stomach acid/reflux for the last 7 or 8 years. I'm hopeful and anxious to get started.

I can't answer regarding the previous repair- but, Dr. Aceves only requires a one week pre-op if your BMI is over 40.. otherwise, you can schedule when you are ready. 

Contact Nina at his office via this email and ask these questions regarding your hernia repair- she can relay that to the Dr directly and get back to you.. that would give you a concrete answer..

Thanks Jo777.  Dr Aceves is the surgeon that I want to use.  I have spoken to Nina at length. At first she was uncertain, but checked and said he could do it.  All good.  Still I'd love to talk to someone in my situation so I don't feel like I'm the first one.  We'll see how this unfolds.  I'm anxious to lose this weight and feel the same joy that people on this board have experienced.  Cheers!
