The individual must have actively participated in non-surgical methods of weight reduction; these efforts must be fully

appraised by the physician requesting authorization for surgery

I have Aetna so it may not be the same, but I just told my PCP about all the past attempts I hade made using Weigh****chers, Slimfast, low carb, going to the gym etc. He put it all in the letter of recommendation. Plus he was the dr. that supervised my 6-month diet plan as well. Everything helps I think.

I have the same insurance, and my surgeon dealt with this in his letter; I didn't have to address it myself.

What, another 6 months on top of what I've already done throughout my life?  No.
