Self Pay Patiens?
I was self-pay with Dr. Kim at Celebration Hospital south of Orlando. It cost about $18K, and it was worth every penny. I don't regret it a bit. I would look closer to home and be less concerned with price and more with the number of sleeves performed, long-term follow-up and support, etc.
My price includes follow up visits with the doctor, nutritionist, exercise physiologist, support groups, and visits with the mental health counselor if needed. I went through surgery consults, info sessions, online orientation, and a full day of orientation class pre-op. My plan is clearly laid out in a binder provided by the surgeon with every question you've ever seen asked on this site pretty much answered -- what/how much/and how to eat, supplements to take, water intake, protein, exercise, constipation, leaks, DVT prevention, and so on.
Good luck!

Good Luck!
.
Oklahoma City (Edmond actually, just North of OKC) has reputable surgeons and the price is around $10,000, but they're probably a half-day drive from you.
Their OH links are here:

They have videos on the links giving background on how they came to choose bariatrics.
Also, their personal website showing the price (there are also other nominal fees)

I personally know several people who have had surgery with them and are very pleased. I just had my first visit with Dr. Walton and feel confident in my choice though choosing which doctor was tough... I understand they assist each other anyway, so six of one, half a dozen of the other?!!!
Good luck in your search!
