any one in illinois who had band to sleeve please help

any one in illinois have a conversion to sleeve after lap band
what physician did you use
how long did it take for insurance to approve or how much out of pocket
was it done in one surgery

 try Alexander Nagle at Northestern hospital in chicago. Having my sleeve soon with him and he seems great...Let me know what you think
