question for RNYs: stretching the pouch/stoma
I think drinking with meals (or too soon after) is a slippery slope because it doesn't seem to impact weight loss much in the early days but lots of people who come back to OH after an absence to ask for help with regain often mention they are drinking while eating.

From what I understand, stetching the pouch is not easy without repeated routine pouch abuse. I'm talking about frequent carbonated drinks, regular drinking while eating, habitual overeating (keeping the pouch stuffed not grazing), etc over time. It is especially damaging early out. We can expect to hold about 8 oz in the long haul if we do what we've committed to doing.
I know less about the stoma. Mechanical failure can cause it to stretch but that's all I know.

based on my surgeon, my nut, and varying things i'm hearing here (and the same post over on the RNY board), i tentatively conclude:
1) stoma and pouch stretching is possible, but extremely unlikely without chronic overeating of non-slider foods.
2) drinking with meals may lead to hunger and therefore overeating, but it seems unlikely to lead to stretching of stoma, bearing in mind point #1.
3) carbonation is to be discouraged early on, causes problems with calcium absorption, and for those addicted to sodas (i have met a few) can lead to serious overconsumption of calories, but in small quantities are not really to be feared (with due respect to dietary beliefs and clean eating practices)
i have been reading also that once the pouch is stretched, if it is in fact stretched, the only option is some type of revision surgery; it will not "bounce back" on it's own. Not speaking to temporary stretching from an one ill-advised meal, but the (apparently) unusual stretching due to long-term pouch abuse. if anyone has information to the contrary i'd be interested to hear.

I had my RNY in Janurary 2002 and lost 115 lbs. I had the "normal stretching" as expected over time after my RNY. I maintained my weight loss for 5 years and my pouch was consistently the same size over that period of time....and I was following the RNY protocol as well.
It was not UNTIL I started changing my diet & drinking habits that my pouch/stoma stretched causing a gain of 50+ lbs over a period of 2 years. The changes were: I started drinking soda & other carbonated drinks which I simply did not do the first 5 years. I started drinking during and after my meals. I ate to the point of discomfort sometimes....all of which gradually stretch your pouch & stoma.
These changes indeed caused my pouch/stoma to stretch. Here is your proof: I followed the protocol strictly for 5 years...NO WEIGHT GAIN and/or changes in amounts I could eat. Then, I changed a few things in my regimen as mentioned above for 2 years...MAJOR WEIGHT GAIN and irreversible STRETCHED STOMA/POUCH. I tried going back to the basics, tried the 5 day pouch test, tried EVERYTHING to no avail. The damage was done and my revision was necessary.
Thankfully, I have another chance at this and I have my mind right this time. I have lost all of the weight I had gained and have met & exceeded my goal weight. :)
