What I've been hearing/doing to prepare!

Sep 21, 2010

Ok so far I went to the WLS seminar, had my New Patient Education Day, as well as my first nutritionalist visit.  I loved Monica, the NUT.  She was really down-to-earth and seems to really listen and be generally interested in your story.  I love that.   I have been a Type 1 diabetic for 30 years now, so I have been checking my BS daily before each meal and at bedtime, and any other time I feel too high or too low.  Monica asked me to start keeping a food journal, which is actually easier than I thought it would be.  I downloaded the My Fitness Pal app onto my iphone and just enter everything I ate for the day before bed each night.  The only problem is that my endocrinologist recently put me on U-500, a concentrated regular insulin:  1 unit of U-500 equals 5 units of regular or novolog, or humalog.  He did this because I was taking about 90 units of Novolog and 70 units of Lantus each day before that. My blood sugars have been running REALLY low lately in the afternoon and evenings and I feel like I'm eating and eating to keep them normal. UGH.  The food log is filled!  I would rather try to lose BEFORE surgery, since they said that they like you to lose 10% of your excess weight before.

FIRST thing is first though, I need to quit smoking. YIKES.  Granted, I only smoke 4-6 cigarettes a day, but still...I need to quit.  In fact, they won't do surgery unless I have.  They actually run a test before surgery to see if you've smoked in the last 2 weeks, supposedly it will show up on the test.  I wanted to quit before my next appt, which is Oct. 7, with my nutritionalist, and I also made an appt that same day with my surgeon because I haven't had the chance to talk to him one on one yet, and I have lots of questions!

I have been researching all of this for like 6 months.  I originally started searching the net for other Type 1's who have had WLS.  There isn't a lot of info about us, the minority!  People tend to look at Type 1 as "juvenile diabetes", as they used to call it when I was diagnosed.  We are not kids anymore!  :))  I have found a few stories about Type 1's that have had RNY and went into ketoacidosis and nearly died because they didn't get enough carbs? I don't really understand it all. But it worries me. It also REALLY worries me that my blood sugars will drop, like they do now, and then I will drink juice to bring them back up, but then throw up from dumping syndrome, bottoming me out and possibly leaving me unconscious!  Wow. Scary.  Soooooooo....I have decided that the sleeve (VSG) is my best option, but everyone at the WL clinic is recommending the RNY to all diabetics!  THIS is the main reason why I need to visit with my surgeon. Monica mentioned leaks and said that's why the PA there doesn't like the sleeve.  Well I need to know what Dr. English will say and how many people he's sleeved and how many have leaked!

The LATEST DRAMA I've encountered (after Monica told me I wouldn't need any other clearances for surgery from other doctors) is that I had an echocardiogram come back abnormal. I called my doctor for the results and he told me it shows that I have a slightly enlarged heart. I asked what that means and he tells me that it's mild diastolic dysfunction. I asked if it was really dangerous. He said the heart normally beats, then rests, beats, then rests. Supposedly my heart doesn't seem to be resting enough. Well what does this mean? I am waiting to see!  I'm really worried about it.  I am young...has my diabetes done this to me? The hypertension? The sleep apnea? The high pulse? GOD knows...I am sure I will need to see a cardiologist for this, and probably need to be cleared for surgery from him.  But first and foremost, I want to know how to fix this, or improve it if I can.  I'm too young for this!  Please someone, tell me it's fixable with diet and exercise!! Sigh. :)  To be continued....
